Required Forms
Health Form and Insurance
Before you'll be allowed to register for classes, you must complete and return the mandatory health form . The form requires your physician's signature, and may require a physical or immunizations.
Health services are provided for students through Syracuse University Health Services through payment of mandatory health fees. It is strongly recommended that students carry health insurance to cover expenses not covered by the health fee and medical expenses incurred outside the Health Center, such as emergency room care, hospitalization, eye care, and dental services.
For information about Health Services and student insurance, please go to Information for New Students.
Degree Bearing Transcript
If you're completing undergraduate studies elsewhere right now, we'll need a copy of a degree-bearing transcript before you'll be allowed to register at SU. Remember to put in the request with your current school's registrar's office before you leave campus. You should request that the transcript be mailed directly to us at:
Graduate Records Office
330 Newhouse 2
215 University Place
Syracuse, NY 13244
Please notify us immediately if it will not be available until after June 30.
Net ID
Once you have sent in your advance deposit and Intent to Register form you should activate your SU Net ID. This will give you access to the University’s e-mail and registration system. It is essential that you activate and check your e-mail account regularly, as it will be the primary communication source for class, program and school information before and after you arrive on campus.
To activate your Net ID, please visit the following link: http://its.syr.edu/sucomputing/.
Your email may be accessed at http://mymail.syr.edu and your personal record, including grades and financial aid, may be accessed at http://myslice.syr.edu.
In mid-May, once we know who will be attending, we will set up a new student listserv using your Syracuse e-mail address (your NetID@syr.edu). It will serve as a means for us to communicate with you as well as a way for you to ask questions of us or of your new classmates, giving you the opportunity to get to know one another.
Summer Course Registration
Around May 15, after you've sent in your deposit and have been officially matriculated, we'll mail you registration information for summer classes. It will include registration instructions and deadlines. At that time we hope to have the Course Information section updated with class descriptions, times, and advance assignments.
